原因

患者精神緊張，針刺入后，局部肌肉強烈收縮；或行針時向同一方向連續(xù)捻轉(zhuǎn)，而致肌纖維纏繞針身；或因針身刺入肌腱；或針刺后體位改變，都可引起滯針。

Cause

This may result from nervousness, strong spasm of the local muscle after the insertion of the needle, twirling the needle with too large amplitude or in one direction only causing muscle fibers to bind, or from a change of the position of the patient after the insertion of the needles.醫(yī)學(xué).全在線www.med126.com

現(xiàn)象

進(jìn)針后針下異常緊澀，出現(xiàn)提插、捻轉(zhuǎn)及出針困難。

Manifestations

After the needle is inserted, it is found difficult or impossible to rotate, lift and thrust the needles.

處理

囑患者消除緊張情緒，使局部肌肉放松，因單向捻轉(zhuǎn)而致者，則須反向捻轉(zhuǎn)。如屬肌肉一時性緊張，可留針一段時間，然后再行捻轉(zhuǎn)出針，也可輕輕按揉針刺周圍的皮膚肌肉，或在附近部位加刺一針，以宣散氣血，緩解痙攣，因體位改變者，應(yīng)恢復(fù)體位后隨之將針取出。

Management

Ask the patient to relax. If the needle is stuck due to excessive rotation in one direction, the condition will release when the needle is twirled in the opposite direction. If stuck needle is caused by temporary muscle spasm, leave the needle in place for a while, then withdraw it by rotating, or by inserting another needle nearby to disperse the Qi and blood, and to relieve the spasm. IF the stuck needle is caused by the changing of the position of the patient, the original posture should be resumed and then withdraw the needle.

預(yù)防

對精神緊張者，下做好解釋工作，消除緊張心理。行針時不可用力過猛，避免單向捻針及將針刺入肌腱，留針過程中不宜隨意改變體位。

Prevention

Nervous patients should be encouraged to relax their tension. Manipulation should not be too forceful. Avoid puncturing the muscular tendon during insertion. Twirling the needle in only one direction shall not be allowed. During retention of the needles, the posture of the patient should remain unchanged.
